Care Transitions HCAHPS Domain
Unplanned readmissions cost the U.S. healthcare system billions of dollars annually. Researchers have identified the discharge process to reduce readmissions. The Care Transitions HCAHPS domain was added in July 2013 to measure patient satisfaction on how well the hospital prepared the patient for their transition from the hospital.

iCAREPASSPORTTM FOR ONGOING PATIENT ENGAGEMENT
HOI uses iCarePassport, powered by Skylight Healthcare Systems, to increase patient understanding on preadmission, during hospitalization and after discharge. iCarePassport’s web-based system enhances the patient experience by defining the process of care and educating patients to be more active participants in their health care.
The interactive iCarePassport system allows discharged patients to report functional status securely and easily in the comfort of their homes. Since its implementation in June 2011, data
on functional status – before surgery and then at various intervals post-surgically – has been captured on the following charts.
